For a start, a little excursion into history.
Many people think that Sklif is an old semicircular building on Bolshaya Sukharevskaya Square in Moscow. That is so, but this is only a small part of today's scientific research institute. N.V. Sklifosovsky.
Officially opened in 1810 by Count Sheremetyev, the Stranger's House at that time was a hospital for "suffering from ailments" for 50 beds and a shelter for girls for 25 beds. It was one of the first hospitals in Russia for the poorest segments of the population. During the Patriotic War of 1812, the hospital was located here, first by the French, then by the Russian armies, and later by the hospital for the wounded in the Russian-Turkish war of 1878.
In 1923, on the basis of the now Sheremetev Hospital, the Institute of Emergency Care was organized, bearing the name N.V. Sklifosovsky. It began the creation of emergency surgical care in the USSR and it was in Sklif that the most complex surgical operations were performed in the Union, when the doctors managed to see what seemed impossible.

This anti-shock department (not to be confused with resuscitation) is the first frontier in the struggle for the life of “critical” patients. As a rule, it is here that patients with serious herbs that can interrupt their lives fall.
The analysis of mortality statistics of victims of polytrauma suggests that shock and blood loss are the main causes of death from injuries in medical institutions, and the measures taken to effectively treat shock were insufficient and not timely. Moreover, some patients could be saved if in the first hours after hospitalization timely diagnostics and treatment of patients were organized - that is why in emergency medicine there is the concept of “golden hour” (Golden Hour in Shock). This is the period of time required for primary diagnostic and therapeutic measures.
Primary diagnosis, as well as stabilization of vital functions as an initial measure, should be made during this hour to avoid prolongation of shock and, thus, later complications. This can be done only with the interaction of an effective team of specialists and with a small expenditure of time for treatment, in a well-equipped anti-shock chamber.
In the antishock department, three tasks are solved:
one.Preservation or restoration of vital functions (control of cardiovascular activity, artificial respiration, infusion and transfusion therapy).
2. Primary diagnosis (radiography, computed tomography, sonography, laboratory diagnosis).
3. Carrying out life-saving operations (intubation, drainage of the pleural cavity, emergency thoracotomy, tracheostomy).
After the vital functions of a person are restored, he is sent either for surgery, if required, or transferred to a specialized department for further treatment and observation.

The most powerful resuscitation service in Russia, consisting of 12 departments, has been created in Sklife today. During the year, more than 8.5 to 10 thousand patients with craniocerebral and concomitant injuries, burns, acute poisonings, diseases of the heart, blood vessels, kidneys, liver, and the gastrointestinal tract undergo intensive therapy. In a special department, treatment is carried out for pronounced clinical and laboratory manifestations of endotoxicosis, developing as a result of multiorgan failure.
